Questions about SonarMD.

Don’t see your question? A care coordinator can help, Monday–Friday, 8 a.m.–5 p.m. CT.

SonarMD is a program for people living with Crohn’s disease or ulcerative colitis (UC). It helps you stay ahead of changes in your gut health and stay connected to your GI care team between visits. Once a month, you answer a short health check-in about your symptoms. Our care coordination team tracks your answers over time. If your symptoms may be getting worse, they connect you with your GI specialist so problems can be caught early.

Signing up takes just a few minutes. If your GI practice offers SonarMD, we’ll send you an invitation with a secure enrollment link via text and email. You may also get information about the program at your next visit.

That’s a great time to start. Many people with Crohn’s or UC live with symptoms they’ve come to see as “normal.” Monthly check-ins give your care team a clear picture of your baseline, even when you haven’t had a recent flare-up. If something changes, they can spot it sooner and help you act early.

Once a month, you get a short health assessment about your symptoms by HIPAA-secure text or email. The questions are based on well-established, validated surveys that GI doctors use to measure Crohn’s and colitis activity. Most people finish in just a few minutes. Your information goes only to your care team, so they can respond if you have a health problem. To learn more, read our Privacy Policy.

SonarMD is completely free to participating patients thanks to coverage through your insurance plan. We will reach out directly to you if you are eligible for the program, and will let you know if for any reason you become ineligible. You may receive an Explanation of Benefits (EOB), which is simply a notice of your coverage. You are not being charged and SonarMD does not affect your deductible.

No, SonarMD does not replace your patient portal. We encourage patients to use their patient portal for all scheduling requests, refills, and test results.

When you enroll, you get access to free partner programs that support your whole health alongside your GI care, including mental, nutritional, sleep, fitness and emotional wellbeing. Explore lifestyle programs here.

“

I really like the checkups monthly to make sure everything is going well, and knowing that my answers are actually being looked at and someone will follow up based on them.

I like having that monthly check in. It’s being more proactive than reactive. If health issues do arise, they can be dealt with before things get out of hand.
