Patients will receive a short health assessment every month that they can respond to via secure email or text. The questions asked on the survey are based on well-established, validated surveys like the Crohn’s Disease Activity Index, Mayo Score and Ulcerative Colitis Disease Activity Index.

SonarMD’s proprietary algorithm then generates a personalized Sonar Score—a numerical representation of patients’ responses. Patients DO NOT see their Sonar Scores, but they do have access to FAQ answers to help them better understand the program.

Patients are asked to respond to the health assessment once a month.

The goal of SonarMD is to bring the patient to your care earlier in the process of their decline than would occur if we waited for the patient to realize that they are deteriorating. Therefore, when our algorithms reveal that a patient is showing symptoms of clinical decline, the SonarMD care coordination team will reach out to the patient to obtain details of the decline. Your practice will then receive these details via a SonarMD alert system, which will facilitate your practice to respond directly – and quickly – to the patient. You will address the needs of your patient no differently from how you would if the patient contacted you directly. We do not intend to disintermediate your doctor-patient relationship, and seek to allow you to continue to enjoy your current workflow.

Your practice will have full access to the SonarMD platform. Accordingly, we will educate your designated staff members on how to operate the platform. Although all physicians can also access the SonarMD platform, we do not expect them to learn how to interpret or use the information contained within the platform.

It is our intention to make the enrollment process as seamless as possible. It is not our intent to disintermediate you from the process. Therefore, we will provide your practice with materials that you can distribute to your interested patients.

There is no charge to the patient for use of the SonarMD platform.

The SonarMD platform continuously monitors patient Sonar Scores. It not only plots the current score, but also the trend of scores over time. When benchmarks in either of these measurements have been exceeded, the SonarMD care coordination team will contact the patient to obtain more detailed information. Your practice will then receive these details via a SonarMD alert system, which will facilitate the practice’s ability to respond directly-and quickly-to the patient.

Yes. SonarMD will send periodic reports containing information on how many patients have enrolled, and their response rate to the monthly surveys. We will also send you periodic reports on how your practice is performing compared to other SonarMD-contracted practices.

From our years of experience operating this program within practices, we learned where administrative burden can be shifted and how to provide practices with key information at the times they need it. SonarMD will enroll, assess, and monitor your patients’ Sonar Scores. We only involve your staff if a patient shows the need for further intervention. By managing the population more closely, you will likely see an increase in your office/virtual visits, but it is our hope and intent that you should concurrently realize a decrease in ER visits and unnecessary hospitalizations.

SonarMD does not integrate into EMRs at this time.

Although the SonarMD platform does not integrate into EMRs, there is no need for you as a provider to use the platform. The platform is intended to be used by your staff.

SonarMD will keep your patient population healthier and out of the hospital. For your involvement in coordinating care for this population of patients, you will receive a share of the savings achieved.

Practices are not subject to any downside risk. In fact, in addition to the shared savings, your practice will receive a prepayment on its portion of shared savings in the form of a per member, per quarter fee. That prepayment does not have to be paid back, even if shared savings do not exceed what has already been paid

Your fee-for-service payments from insurers remain the same. The payment from SonarMD is added on to your typical reimbursement.
