About Us

SonarMD is the leading value-based care coordination solution for complex chronic diseases.

Our longitudinal, high-engagement platform makes it easy for specialists and patients to work together to detect deteriorating health sooner — and intervene faster — to prevent costly health emergencies. We use a risk-based model to align clinical and financial incentives, sharing savings with health insurers and specialists to lower the overall cost of care.

Our first focus is improving care coordination for gastrointestinal disease, which has an overall annual health expenditure totaling $136 billion. We’ve proven we can keep people with inflammatory bowel disease, one of the biggest drivers of cost, healthier and reduce costs by 15% per member per year.
